Who it's for

Men and women aged 18 and over who:

  • are stepping down from one of our own 24-hour homes
  • are referred from other providers or from hospital, and need less support
  • are getting ready for their own tenancy
  • are young adults moving on, including from Greatfields, our service for 18 to 25-year-olds

Our studios

Our move-on homes give people a place of their own to practise living independently, with our team close by.

  • Hounslow House, Hounslow: 10 self-contained studio flats for men and women, with low / move-on support.
  • Rosewood House, Hounslow: one self-contained flat and four studios, with low to medium support.

How we support people

  • Regular keyworking. Planned one-to-one sessions every week.
  • Life skills. Budgeting, cooking, bills and how to manage a tenancy.
  • Staff close by. A team nearby during the day, and an out-of-hours route.
  • The same specialist team. People keep access to our in-house psychology, nutrition and activities team.

What we work towards

  • A home of their own: council, housing association or private rented.
  • Staying well and living independently, without returning to crisis or hospital.
  • Work, study or volunteering.
  • No cliff edge: moving on without losing the people who know them.

After you leave a home

  • Help finding a tenancy. We help people find a home of their own, whether council, housing association or private rented.
  • Settling-in visits. Our staff visit for a while after the move, to help people settle into their new home.
  • An open door. If things get hard, people can come back to us.

Of the 33 people who moved on in the past year, 20 went to other supported housing, their own tenancy, family or university.

Moves between our own services are not counted. Foxglove is not included. October 2025 to September 2026. How we measure this

What we do differently

  • Specialists included. Our own psychology, nutrition and activities team is part of one weekly rate, with no bolt-on charges.
  • A full pathway with one provider. People move from 24-hour support to lower support and on to our own move-on studio flats, keeping the team who know them.
  • Fast, straight answers. Our median time from referral to assessment is 3 days. If a placement isn't right, we tell you, and we explain why.
  • Digital and transparent. Our records are digital, and we share service-level reports with placing authorities.

Our commitments

  • Everyone has a move-on plan from the day they arrive.
  • We track placement stability, move-on and readmissions for everyone we support, and share service-level reports with placing authorities.
  • If a placement isn't right, we say so, and explain why.
  • When we find something that works, we share it with the sector.

Leading the change

Our goal is simple: nobody stuck. No one should stay in a service longer than they need to. To get there:

  • We are opening more specialist services.
  • We are growing into new boroughs.
  • As our data builds, we will publish our outcomes openly.

Can people refer in from outside Diverse Services?

Yes. We accept referrals from other providers and from hospital for people who need low or medium support.

What's the difference between Hounslow House and Rosewood House?

Hounslow House has ten self-contained studio flats. Rosewood House has one self-contained flat and four studios. Hounslow House offers low / move-on support and Rosewood House offers low to medium support. Both are for men and women.

What happens when someone moves into their own home?

We plan the move with them from day one, so moving on doesn't mean losing the people who know them.
