Stage 1 of 4

24-hour high support

Adults with severe and enduring mental health needs who need staff on hand day and night, including people stepping down from hospital.

What support looks like

Staff on site 24 hours, a keyworker with weekly keyworking sessions towards agreed goals, daily support with everyday living, and our in-house psychology, nutrition and activities team.

What the person works on

Feeling safe and settled, daily routines, medication, and a risk management plan that works.

Ready for the next step when

Overnight support is no longer needed, and the person is keen to do more for themselves.

Recovery isn't a straight line. Because we cover every step from 24-hour support to independence, nobody has to start again with strangers.

We help you take the next step

  1. Your keyworker agrees the next goal with you, at your pace.
  2. You can step down from 24-hour support to lower support, then to one of our own studio flats.
  3. When you're ready for your own tenancy, we help you find it and visit after you move.

Can I refer someone straight into move-on accommodation?

Yes. We accept referrals from other providers and from hospital for people who need low or medium support.

Do people have to leave Diverse Services to move on?

No. People can move from 24-hour support to lower support and on to our own move-on studio flats, keeping the team who know them.

Which councils and NHS teams refer to us?

We currently support people placed by Ealing, Hammersmith & Fulham, Hillingdon, Hounslow, Kensington & Chelsea, Westminster, Barnet, Croydon and Redbridge councils, and NHS North West London ICB.

Why the missing step matters

Many people stay in high-cost 24-hour support simply because there is nowhere in between to move to. Our lower-support homes and move-on studio flats give people that next step, with the same team.

  • Of the 33 people who moved on in the past year, 20 went to other supported housing, their own tenancy, family or university. (October 2025 to September 2026. Moves between our own services are not counted. Foxglove is not included.)
  • Median length of stay so far: 18 months. (As at 1 October 2026.)
