We support adults with mental health needs across Hammersmith & Fulham, Hounslow, Hillingdon and Ealing. Seven in ten of the people we support live with schizophrenia or psychosis. Each of our services is built for a particular group of people, and all of them sit on one pathway. As people need less support, they can move on without starting again with strangers.

24-hour high support
Adults with severe and enduring mental health needs who need staff on hand day and night, including people stepping down from hospital.
What support looks like
Staff on site 24 hours, a keyworker with weekly keyworking sessions towards agreed goals, daily support with everyday living, and our in-house psychology, nutrition and activities team.
What the person works on
Feeling safe and settled, daily routines, medication, and a risk management plan that works.
Ready for the next step when
Overnight support is no longer needed, and the person is keen to do more for themselves.

What a day can look like
Every day is shaped around the person, their goals and how they are feeling. A day in one of our homes might include:
- Morning
Help with everyday living, such as medication, getting ready and planning the day, with staff on hand.
- Keyworking
Time with a keyworker, working towards the goals the person has agreed.
- Groups and 1 to 1
Group sessions or 1 to 1 time with our in-house psychology team.
- Food
Nutrition sessions or weekly group cooking with our Nutritionist.
- Afternoon
Activities, outings, life skills sessions, art and music, often led by the people we support.
- Evening and night
In 24-hour homes, staff are on site through the evening and overnight.
Who is in the team
Support staff
Our own employed team, with zero agency staff. Each person has a keyworker and weekly keyworking sessions.
Psychology
A Psychology Lead and four Assistant Psychologists, offering risk formulation, group sessions and 1 to 1 work where people live.
Nutrition
Our in-house Nutritionist, with tailored dietary planning, nutrition education and weekly group cooking.
Activities
Our Activities Coordinator, a Housing with Care Awards 2026 finalist, runs a weekly programme of groups, outings and life skills.
What we do differently
- Specialists included. Our own psychology, nutrition and activities team is part of one weekly rate, with no bolt-on charges.
- A full pathway with one provider. People move from 24-hour support to lower support and on to our own move-on studio flats, keeping the team who know them.
- Fast, straight answers. Our median time from referral to assessment is 3 days. If a placement isn't right, we tell you, and we explain why.
- Digital and transparent. Our records are digital, and we share service-level reports with placing authorities.

Our median time from referral to assessment is 3 days, and 82% of people are assessed within a week.
Visit
The person and the people close to them can visit the home before deciding.

Everyone has a move-on plan from the day they arrive.

Professionals, such as care coordinators and social workers, and families can refer online at any time.
Can we visit before deciding?
Yes. Use the Request a visit button and our team will arrange a time.
Are the specialist team extra?
No. Our psychology, nutrition and activities team is part of one weekly rate, with no bolt-on charges.
How long do people stay?
As long as they need, and no longer. Everyone has a move-on plan from the day they arrive.
